SELF REFERRAL SERVICES AVAILABLE

There are many community services which can be accessed without needing to speak to a doctor.  A self referral can be done by simply contacting the call centre in your area and asking to be put through to the relevant department.

• Incontinence team - self referral
• Occupational Therapy - self referral
• Podiatry - self referral
• Speech and language - self referral
• Social care (care packages etc) - self referral
• Physiotherapy - self referral
• District Nursing - to be arranged by GP or medical professional
