Sick notes

To request a sick note:

• phone or visit the surgery after 10.30am on any weekday morning and request a sickline from the GP. 
• You will need to inform reception of the date your sickline is starting, for how long you require it and what the reason is.
• Please note sicklines cannot be dated in the future.
• If need a extension to your existing sickline, this will start on the day after your existing one ends. We are unable to request an extension until your existing one has ended.

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
