Join the surgery

If you need to register with a GP, please complete the relevant form for you below. 

• Patients wishing to transfer GP within Northern Ireland or transferring from Great Britain need to complete a form HS200.

• Eligible visitors or persons taking up ordinary residence in Northern Ireland need to complete a form HSCR-1.

• You can call into the Surgery any time during opening hours and pick up the appropriate form

This is an automated system, and all requests will be processed in English. If you require your medical card in another language, please contact the BSO on the following number 0300 555 0113 where you will be advised on how to make your request.

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

• Patients do not need to see their named GP when they book an appointment with the practice.
• Patients are entitled to choose to see any GP or nurse in the practice, where available.
